Midcontinent Foundation Grant Application

 

Please use the form below to submit your grant request.  An endorsement by a Midcontinent Communications employee is preferred and proof of your organization’s IRS 501C3 Certificate of Designation is required to be considered for a grant. The Foundation reviews all grants in March and September of each year.

 

The Spring 2013 grant application window is now closed.  We will be accepting applications for Fall 2013 grants through July 10, 2013. 

 

Please ensure that all attached documents are valid files that can be transferred electronically. If you have any questions, please contact us for assistance. Funding is distributed shortly after Foundation approval.
